In-store and online purchases associated with your ExtraCare card will count toward your rewards. All you have to do is complete the short request form to start earning double quarterly ExtraBuck Rewards.

Quarterly ExtraBucks Rewards will be available on your store receipt, ExtraCare Coupon Center or CVS.com 01/01/13 – 02/14/13.
